Redefining the Coordination for VTE - Site of Care, Clinicians, and Patients: Part 1

This EMCREG-International On the Go program serves as a sharing of interdisciplinary best practices of how to optimize and improve the current management of venous thromboembolism (VTE) and the consistent transition of care for practicing physicians, nurses, and pharmacists. The faculty members presenting in this first of 2 programs are specialists in the field of emergency medicine, hospital medicine, pharmacy, and hematology. The focus will 3-fold: (1) profiling best practices in the diagnosis and initiation of VTE therapy, (2) coordinating anticoagulation care transitioning from the emergency department and observation units during the COVID-19 pandemic, and (3) discharge planning.
